A Disabled Facilities Grant (DFG) - GOV.UK is a grant to help people with disabilities to adapt their homes to enable them to continue living there.
Home adaptations for under 18s
Please contact the children’s Occupational Therapy service (SWFT) to request support and/or assessment. If appropriate, Occupational Therapy will refer through to HEART who administer the DFG for all Warwickshire residents.
If HEART are able to issue a grant but the cost of adaptations exceeds their funding limit, a Family Help Assessment may be required to explore the option of top-up funding. HEART will explore a variety of options and discuss the process with you.
If a young person has an allocated social worker, it is advised they discuss the need for home adaptations with them. The allocated worker will then be able to support with next steps and an Occupational Therapy referral.
Home adaptations for over 18s
Over 18s can contact HEART directly to request an assessment for a Disabled Facility Grant to support with home adaptations.
If HEART can issue a grant but the cost of adaptations exceeds their funding limit, they will explore a range of options for additional funding and discuss this with you.
