Reviewing your care and support
We will arrange review meetings with you to look at your care and support plan and see if the support you are receiving is still helping you to achieve your outcomes.
The review will look at your plan to consider what’s working, what’s not working and what might need to change. We do this so that we can make sure that the plan to meet your needs is up-to-date and completely right for you. Reviewing your care helps to prevent problems before they occur by checking that the plan is working well in meeting your needs and that it enables you to continue to do the things you can do for yourself. A review will also help to find out those things that are important to you and try to seek ways to help you achieve them.
We will check at your review whether your personal budget is at the right amount to meet your needs. A personal budget is the amount that the council decides is right to meet your care and support needs. The amount is decided after an assessment has been completed. The review may result in the budget staying the same, increasing or being reduced. In some circumstances, it may be that once the review is completed, a reassessment of your needs is required, which we will complete with you.
The review will involve the person receiving care and support and the carer where possible. Sometimes, it may be difficult for the person to contribute and therefore a representative or independent advocate may need to be involved. If an independent advocate is needed, we can arrange this for you. The review will also involve talking to those that are providing the care and support you are receiving.
Reviews can happen in a variety of different ways.
- Planned - usually having an identified future date
- Unplanned - can happen when we find out that circumstances have suddenly changed
- Requested - when you, a family member, representative or someone else who supports you, tells us that you have had a change in needs or circumstances.
You can tell us at any time if you feel your needs have changed.
