Parent Carer Support Pathway

Typically, Parent Carers' needs will be assessed as part of the Family Help Assessment, leading to a holistic support plan for your entire family. As a parent carer, you can request a separate Parent Carers Pathway.

  • If you have a Social Worker, Family Practitioner or Family Support Worker: please contact them to discuss this further.
  • If you do not have an allocated worker: you can get in touch with the Children with Disabilities Team by completing the online contact form with your name and contact details, and one of our practitioners will reach out to discuss your needs. Alternatively, you can call us at 01926 413737 and select option 4 (available Monday to Thursday 9am – 5:30pm and Friday 9am – 5pm) to start the Parent Carer Support Pathway.

Depending on service demands, the Wellbeing Conversation can be held over the phone immediately and may take up to an hour. If this isn't possible, we can arrange a convenient time to call you back.

The assessment will look at:

  • Your needs for support as a parent carer
  • Your wellbeing
  • Factors that could make caring for your child easier
  • Whether it's appropriate for you to continue providing care for your disabled child
  • Safeguarding and promoting the welfare of your disabled child and any other children you have.
