Community Recovery Service Programme
The CRS Programme aims to provide Warwickshire residents with increased opportunities for ‘home first’ support following hospital discharge. It establishes a single point of access, enabling timely discharge from hospital and facilitating home-based assessment for a greater number of individuals. Therapy support is available throughout the assessment of individuals’ longer-term care needs, with an ongoing ambition to discharge more people within 24 hours of referral.
This approach enables individuals to access a combination of domiciliary care and therapy support at home, provided free of charge for up to six weeks, supporting recovery, independence, and reablement following hospital admission. To support this model, the Council continues to commission domiciliary care in a different way through the use of pre-paid blocks of care, ensuring flexibility and maintaining a strong reablement ethos focused on supporting people at home rather than in hospital settings.
The accompanying graph shows the number of CRS hours delivered across zones2024/25. The data demonstrates a consistent pattern of demand, with Zone 3 remaining the most common area for CRS referrals, followed by Zone 4 and Zone 2.
