How can I be referred?

Healthcare or social care professionals can then make a referral to the scheme on behalf of their patients. This can be done online or by post.

What happens after I have been referred?

Fitter Futures Warwickshire will make contact with you to confirm you have been accepted on the Fitter Futures Warwickshire scheme. They will then forward your details to the relevant service provider. The service provider will then contact you to arrange your first visit.

How much does the scheme cost?

  • Young People and Adult Physical Activity/Healthy Lifestyles on Referral service - The leisure centre based sessions will be provided at a reduced cost. The Physical Activity Consultant service is free of charge for 12 weeks.
  • Young People and Adult Weight Management on Referral service - free of charge for a 12-week programme.
  • Family Weight Management - free of charge for a 9-week programme.
  • Healthy Lifestyles 0-5 service - please contact the service providers for further details
