Description
| The community Occupational Therapist (OT) works as a member of a team, working with adults and older people, and giving advice as to how to access services for children.
The OT visits people in their home who are having difficulty with day to day tasks that they are, or may be:
- at risk of harm,
- at risk of losing their independence,
- at risk of damaging their health and well being.
The OT assesses all an individual's needs and, with the person, they agree a plan of action to achieve independence and safety within their home, work and leisure environments. The needs of carers are also assessed where appropriate.
The OT may:
- suggest alternative ways of completing an activity,
- loan specialist equipment,
- recommend minor building adaptations such as grab rails,
- or recommend grant support for major adaptations to make the home more suitable for independant living.

| Eligibility | To qualify for an OT assessment, the person's needs must meet the Department's Fair Access to Care Standards criterion of having a level of risk that is high or substantial.

| Comments | Advice and information may be given where a person does not meet the department's priority levels of service.
