5. How to apply
Anyone can apply for a copy certificate if they can supply enough information to identify the correct record and pay the fee. However, if we suspect that the application is not for legitimate reasons, we will report it to the General Register Office.
A record is kept of every certificate issued so police and other authorities can be informed if certificate numbers are used for fraud or other crime.
The fee per copy certificate is £10.00.
Please note – only a limited number of records are currently available to order online. We will be adding more records to this system and apologise for any inconvenience. You can still apply for copies of these certificates by post or in person.
Apply in person
You can request certificates at the following registration offices, either in person or postal application.
- Nuneaton registration office – registers for events recorded in north Warwickshire (North Warwickshire Borough and Nuneaton & Bedworth Borough areas).
- Rugby registration office – registers for events recorded in east Warwickshire (Rugby Borough area).
- Warwick registration office – registers for events recorded in south Warwickshire (Warwick District and Stratford upon Avon District areas)
Other registration offices in Warwickshire do not issue birth, death or marriage certificates other than those relating to events which have been registered very recently at that office. If you wish to find out whether the record you require is at Nuneaton, Rugby or Warwick please telephone us. This is particularly important if you are applying for a marriage certificate as some churches still hold and use the original registers.
You do not need to make an appointment to apply for a copy certificate. You will be able to complete an application form and leave the fee. Please note we only accept cash for these applications.
We will advise you when the certificate will be ready for collection or you can request for it to be posted to you. Our offices may be able to offer an express service for an urgent copy certificate, please call in advance to enquire.
This service is not available at our registration offices on a Saturday or Sunday.
Apply by post
Please send completed forms to the appropriate registration office (details on form below). Please include a cheque or postal order for the appropriate amount made payable to Warwickshire County Council along with a stamped addressed envelope so we can send the certificate to you. Please ensure that the postage is correct for the size of the envelope you provide.

You will need:
- full name at birth;
- date and place of birth;
- parent(s) name(s) including mothers maiden name (if appropriate).
Copy death certificateCopy death certificate application form (DOC, 835 KB)
You will need:
- full name of the deceased;
- place and date of death.
Copy marriage or civil partnership certificate
You will need:
- full names of both parties at the time of the marriage/partnership;
- date and place of marriage/partnership.
Applying for a certificate if you are adopted
If you require a copy of your adopted birth entry, you should apply to the: Gov.uk – General Register Office.
