About Adult Services - Older People

If you are an older person and think you might need our support, we will talk to you, your relatives and carers, and other people who are important to you. We will consider your problems and circumstances and what your needs are.

When we have agreed what needs you have, we could provide the following sorts of services:

home care to support you in your own home, possibly through direct payments if this is your choice:
daycare in a day services or in a residential home;
meals service where meals are delivered to your home;
equipment and adaptations to help you keep your independence;
reablement services to help you gain confidence and skills again;
care homes for when you are no longer able to live at home, and
hospital discharge services so you can make plans for the support you will need when you return home from hospital.
