Warwickshire Single Assessment Process (SAP)


What is the Single Assessment Process (SAP)?
It is a new way to share information about your personal circumstances, views, needs and problems. It is mainly for people with complex health and social care needs. Previously, hospitals, nurses, GPs and social care staff (previously Social Services) all did their assessments separately. The single assessment process will save you repeating your details to those who provide you with care and treatment.

What is an Assessment?
An Assessment is how we collect information about your needs, and how they affect your day-to-day life. We can then decide the best way to give you the help or treatment you need. As people's health and social care needs overlap, it obviously works better if we can share such information.

How does it work?
People assessing your needs will put the information into an orange folder, which you will keep. It will help if you show the folder to people who are involved in your care or treatment, or who are doing assessments. This should save you repeating your personal details and your needs or problems to the different people who provide you with care or treatment.

How will you handle my personal information?
We need your consent to share it. We must tell you who we need to share it with and why. You can withdraw your consent at any time. Those who might need to share include your GP, nurse, hospital, social care staff, ambulance service, housing and voluntary organisations, all of whom have signed an Information Sharing Charter that sets rules and standards about sharing confidential information. They have a legal duty to handle your information confidentially and securely in accordance with the Data Protection Act 1998.
