Students who are continuing Higher Education in 2008/2009

Please note: if you have completed a HND or foundation degree and are continuing onto a top up degree, or have completed a degree and are planning on doing a PGCE you should complete a PN1 Application form.

This form can be used to provide evidence of any Sponsors Benefits received in the 2007/2008 Financial Year and must be completed by Jobcentre Plus
Confirmation of Earnings (CI2) - Click here to download CI2 form.
This form can be used to provide evidence of any Sponsors earned income for the 2007/2008 financial year and must be completed by the Sponsors Employer.
Confirmation of Earnings from Self Employment (GSA1) - Click here to download GSA1 form.
This form can be used to confirm a Sponsors Self Employment income, any Property Income or Company Director's Income for nthe 2007/2008 financial year
Change of Circumstances Form for Full Time students (CO1) - Click here to download CO1 form.
This form should be used if a students circumstances change, i.e. suspension, withdrawal, (full time undergraduate student only, if you are a part time student please see relevant section below). Application for Help with Childcare (CCG1) 2008/2009 - Application forms to apply for assistance towards the cost of Childcare should be available towards the end of April.
This form can be used to apply for Childcare Costs you incur whilst studying. Please note that this allowance is mean tested on household income and the student, their spouse or partner must not be in receipt of the childcare element of the Working Tax Credit. An application form must have already been submitted, if you have not submitted an application form for the 2008/2009 academic year please download and return the correct form above.
Confirmation of Childcare Costs (CCG2) 2008/2009 - Application forms to apply for assistance towards the cost of Childcare should be available towards the end of April.
This form is used to confirm the estimated childcare costs submitted on the CCG1 Application Form
Guide to Applying for Assistance with Childcare Costs whilst in Higher Education 2008/2009 - This Guide should be available towards the end of April.

Applying Online - 2008/2009 academic year
You can also make an application online at www.studentfinancedirect.co.uk. You will be required to send all necessary evidence in support of your application to your Local Authority once you have completed your online application. if you are a new student this will include your original, valid Passport. Any other required evidence will depend on your circumstances and the type of support you have applied for.
UK Nationals only: If you do not have a valid passport, you may submit your original birth certificate as long as the birth/adoption certificate declaration form has been completed in full as instructed and submitted along with your birth certificate. Applying for Part Time funding for the 2008/2009 academic year
Form PTG1 and PTG1 notes. This is the DfES Application form for new and continuing students for the 2008/2009 academic year.
This form is for students who are studying an undergraduate course part time.
Please note: students who are repeating part of a full time undergraduate degree part time should complete a PR1 Application Form. Also, students who are studying a part time PGCE or ITT course come under the Full Time Regulations and should complete either a PN1 (new students) or a PR1 (continuing students).
Students applying for part time assistance can not apply online, they must complete a paper form.

Confirmation of Benefits for Part Time Students (CB1) - Click here to download CB1 Form
This form can be used to provide evidence of any State Benefits the student or their partner/spouse received in the 2007/2008 Finanacial Year and must be completed by either the Jobcentre Plus, Social Security Offices, local authority housing or Council Tax Offices, depending on the Benefit received.
