Applying for a Blue Badge

We recommend that you read the Frequently Asked Questions ( FAQ )before applying for a Blue Badge.

You can obtain an Application Pack for a Blue Badge by:

Printing the Application Form on this page and returning it to the County Council Customer Service Centre, Shire Hall, Warwick,CV34 4TH.
Or by contacting the Customer Service Centre and requesting an application form. You can do this
- by telephone - 01926 410410, or by minicom -01926 412277. Opening hours are 8a.m. to 8p.m, Monday to Friday; and 9a.m. to 4p.m.on Saturday
- by letter - to address as above
- by email to cscworkflow@warwickshire.gov.uk
BUT please do not call in person as the service is not set up to receive personal callers.

Your Ethnic Background

These documents are in pdf format. You may need to install Adobe Acrobat Reader to view them.

When applying for a Blue Badge please enclose:

your application form - please remember to sign the application twice, once in Part G and once in the Signature box;
copies of any evidence required to support your application (PLEASE DO NOT SEND ORIGINALS IN THE POST);
two passport-sized photographs of yourself, which you have signed on the back(these can be family photographs cut to size);
fee of £2.00 (please make cheques payable to Warwickshire County Council).

Please remember that fees and photographs will not be returned if your application is unsuccessful.
