Alternatively, you can get an Application Pack for a Blue Badge by contacting the Customer Service Centre. You can do this by:
  • telephone - 01926 410410, or by minicom - 01926 412277. Opening hours are 8a.m. to 8p.m, Monday to Friday; and 9a.m. to 4p.m.on Saturday
  • letter - to the address listed below
  • email to cscworkflow@warwickshire.gov.uk
  • BUT please do not call in person as the service is not set up to receive personal callers.

Your Ethnic Background


When applying for a Blue Badge please enclose:
  • your application form - please remember to sign the application twice, once in Part G and once in the Signature box;
  • copies of any evidence required to support your application (PLEASE DO NOT SEND ORIGINALS IN THE POST);
  • two passport-sized photographs of yourself, which you have signed on the back(these can be family photographs cut to size);
  • fee of £2.00 (please make cheques payable to Warwickshire County Council).

Please remember that fees and photographs will not be returned if your application is unsuccessful.
