The Purpose of the Support

To improve levels of healthier living - to reduce likelihood of hospitalisation and reduce length of stay in hospital;
To make communities safer, reduce offending behaviour, increase safety awareness;
To take part in the community and strengthen communities;
To improve economic well-being.

Services can include a home visit for a short period each week, or an on-site, full-time support worker for a long period of time.
